Winter Athletic Certification Information

 

In an effort to better communicate with parents, I am outlining the process to have your son/daughter ready to try out for a sport this winter. There is a process that needs to be completed before each and every sport season, fall, winter and spring.  This is called the re-certification process. The process to re-certify athletes will be as follows:

 

ü      Students will get the Interscholastic Athletic Form and have parents/students complete and sign. The student may get this in the AD’s office, Nurses office, Main office or their coach.

ü      Student will re-certify with the nurse within 30 days of the upcoming season. The nurse and the athletic director will set up dates and times for this to occur.

ü      All students must be physically seen by the nurse to be re-certified. Dates and times are as follows;

 

  1. High School - 2:15- 3:00 in the High School Nurse’s office
    ·        October 29, 30,31 and November 2nd (Hockey, Cheerleading and Wrestling)
    ·        November 5, 8, 9, and 12th (All Others)

  2. Middle School       
    ·        Girls Cheerleading   11/6, 11/7       2:15-2:45pm
    ·        Girls Basketball       A thru M   11/12       N thru Z   11/13     2:15-2:45pm
    ·        Boys Basketball      A thru M   11/14       N thru Z   11/15     2:15-2:45pm
    ·        Boys Wrestling       11/16       2:15-2:45pm
    ·        Boys Swimming      11/19       2:15-2:45pm 

 

ü      Physicals are good for 1 full year. If the physical falls on the month of the appropriate season, it is good for one full year plus that sport season.

ü      The nurse will complete the re-certification form and distribute it to the appropriate place.

ü      The form:

      1.      The white copy goes into the students medical file

      2.      The gold copy goes to the student athlete. This becomes the student/athletes ticket to the first day of practice.

      3.      The yellow copy goes to the athletic director

      4.      The pink copy goes to the coach.
